What Happens During a Home Care Assessment?

An assessment is simply a conversation about the person, their routine and the support that would help.

An assessment can sound formal. In practice it is a conversation, usually in the person's own home, at a time that suits.

What we talk about Daily routines, what someone can manage independently, where they would like support, health and mobility, medication, food and drink, and anything that matters to them personally, from a preferred morning routine to a favourite chair.

Safety and risk We look at practical things such as moving around the home, bathing, and anything that could cause harm, so support can be planned safely.

What happens next The conversation is used to build an individual care plan setting out what support is provided, when, and how the person prefers it to be delivered. Nothing is fixed forever, and the plan is reviewed as circumstances change.

Who should be there Anyone the person wants there. Many families join the conversation, and that is usually helpful.
